If you live within our practice area you are welcome to register with us and our reception team will be happy to guide you through the procedure.

Holiday Makers and Visitors staying in our practice area are welcome to contact the surgery for an appointment.

Visitors staying for longer than 3 months must register with the practice as a permanent patient.

If you are an overseas visitor not covered by a reciprocal agreement you may be charged a fee of £50.00 for non-emergency treatment.

To register with the Practice you will need to:

  • Print and complete the GMS1 and New Patient Registration forms (attach documents)
  • Ensure you include your 10 digit NHS Number – your previous GP Practice can help with this
  • Use the health monitor in our waiting room to take your height, weight and blood pressure
  • Bring identification documents with you: photo ID and up-to-date proof of your residential address

Then ask the Receptionist to complete the registration (either wait until one is available to assist you, or book a time to do this.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will need to sign this form to confirm all details are correct and provide current photo ID i.e. driving licence or passport.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
